Wellpath Clinic Privacy Policy for Collection, Use, and Disclosure of Personal Information

Privacy of your personal information is an important part of this clinic, while providing you with quality Naturopathic care. Wellpath Clinic recognizes the importance of privacy and the sensitivity of personal information. We are committed to protecting any personal information that we hold. This Privacy Policy outlines how we manage your personal information and safeguard your privacy.

Wellpath Clinic is responsible for the personal information we collect and hold. To ensure this accountability, we have developed this policy and trained our Naturopathic Doctors and support staff about our policies and practices.

At Wellpath Clinic, our privacy policy ensures that:

  • only necessary information is collected about you;
  • we only share your information with your consent;
  • storage, retention, and destruction of your personal information complies with existing legislation and privacy protection protocols; and
  • our privacy protocols comply with privacy legislation and standards of our regulatory body, the Board of Directors of Drugless Therapy - Naturopathy.

What Personal Information do we collect

Personal information is any information that identifies your, or from which your identity could be deduced. It typically includes your name, address, telephone numbers(s), and your email address.

How do we collect your personal information

We collect personal information only by lawful means and not in an unreasonably intrusive way. Wherever possible, we collect your personal information directly from you, at the start of your consultation with our clinic and in the course of providing services and treatment to you. Sometimes we may obtain information about you from other sources, for example, your insurance company, other health care providers, and medical laboratories.

How our Clinic Uses and Discloses Patients Personal Information

Wellpath Clinic will use and disclose personal information about you for the following purposes:

  • to assess your health concerns and provide you with quality health care
  • to advise you of treatment options
  • to establish and maintain contact with you
  • to send you newsletters and other information mailings
  • to remind you of upcoming appointments
  • with your permission, to communicate with other treating health care providers
  • to allow us to efficiently follow-up for treatment, care, and billing matters
  • where it is necessary to collect outstanding fees
  • to comply with our legal and regulatory requirements, and to comply generally with the law
  • if the information is already publicly known

If you tell us that you no longer wish to receive information about our services or our about new developments in naturopathic medicine, we will not send any further material. Wellpath Clinic does not disclose your personal information to any third parties to enable them to market their products and services.

Updating Your Information

Since we use your personal information to provide health care services to you, it is important that the information be accurate and up-to-date. If during the course of the retainer any of your information changes, please inform us so that we can make any necessary changes.

Is Your Personal Information Secure?

Wellpath Clinic takes all reasonable precautions to ensure that your personal information is kept safe from loss, unauthorized access, modification, or disclosure. Among the steps taken to protect your personal information are:

  • Premises security;
  • Restricted file access to personal information;
  • Deploying technological safeguards like security software and firewalls to prevent hacking or unauthorized computer access; and
  • Internal password and security policies.

Access to Your Personal Information

You may ask for access to any personal information we hold about you. Summary information is available on request. More detailed requests that require archive or other retrieval costs may be subject to administration and disbursement fees.

Correcting Errors

If Wellpath Clinic holds information about you and you can establish that it is not accurate, complete and up-to-date, Wellpath Clinic will take reasonable steps to correct it.

Can You Be Denied Access to Your Personal Information?

Your rights to access your personal information are not absolute. We may deny access when:

  • When granting you access would have an unreasonable impact on other people's privacy;
  • To protect our clinic’s rights and property; and
  • Where the request is frivolous or vexatious.

If we deny your request for access to, or refuse a request to correct information, we shall explain why.

Communicating with Us

You should be aware that e-mail is not an entirely secure medium. You should be aware of this when using email to send or receive personal or confidential information.

Changes to this Privacy Policy

Since Wellpath Clinic regularly reviews all of its policies and procedures, we may change our Privacy Policy from time to time. All changes will be posted on our website.
